Position: Aftercare Coordinator
Location: Shelburne & Toronto

Pine River Institute is a Residential Treatment Centre and Outdoor Leadership Experience for youth 13 to 19 struggling with substance abuse and the cluster of behaviours often associated with it. Our campus is located on a 200-acre site 100 kilometers northwest of Toronto, Ontario, and includes a residential program, an 8-bed therapeutic wilderness program near Algonquin Park, and an aftercare program, with an office in Toronto. Our family-centered program is innovative and highly structured, and creates opportunities for troubled teens to engage in a focused transformation process through a fusion of education, therapy, recreation, and life skills. The majority of our students are from Ontario, and are funded by the Ministries of Health and Education.  We also work with families from across Canada, and internationally.

We are currently seeking an AFTERCARE CO-ORDINATOR to start on November 14, 2011.  Estimated 2-3 days a week, with one day/week on campus, and available at least two weekend days/month (Saturday or Sunday) and one evening/month, and evening phone calls to students and families.

POSITION OVERVIEW:

Reporting directly to the Clinical Director, the Aftercare Coordinator is responsible to coach and support the students and the parents as they reintegrate into the community. Success in this role will depend on one’s ability to create a supportive culture that allows for the student and their families to be launched in an effective and responsible manner.  The Aftercare Coordinator ensures that all student, family, and community experiences are at a level of excellence.


RESPONSIBILITIES OF THIS POSITION INCLUDE BUT ARE NOT LIMITED TO:

  1. Manages a case load of 10 to 12 students and supports them as they reintegrate into the community.  Assists them (with the parent’s help) in finding employment, volunteer positions, therapeutic support, recreation activities, and/or academic placements 

  2. Leads weekly Aftercare Preparation group on campus for students in the Transition Phase of the program

  3. Provides weekly check-in calls to parents and students

  4. Co-leads weekly Parent Support Teleconference

  5. Organizes and leads bi-monthly alumni and aftercare student activities

  6. Organizes and leads quarterly alumni and aftercare student workshops

  7. Works closely with the student’s designated mentor and therapist from PRI to support the student in an attuned manner.

  8. Liaise with Research Director on follow-up data collection

  9. Liaise with Communications Director on regular alumni communications

  10. Liaise with community resources—academic, therapeutic, recreational, volunteer opportunities, etc


QUALIFICATIONS FOR THIS POSITION INCLUDE:

  1. Post-secondary degree in relevant field

  2. Ability to communicate effectively with parents and teens

  3. Ability to work with a team of interdisciplinary professionals

  4. Excellent organizational skills

  5. Flexible schedule

  6. Valid driver’s license and car insurance

  7. Familiar with Toronto and GTA based resources and supports

  8. Resourceful in identifying supports in other locations
